Services Provided & Patient Care Policy

We are dedicated to providing the most qualified personnel to the patient in his/her own home in order to maximize and fulfill his/her goals as a member of the community.

Services are available 24 hours a day, seven days a week using an answering service and on-call procedures after normal working hours, weekends, and holidays. As needed, staff may be assigned to make patient visits after normal business hours. We offer skilled nursing services and to the extent that appropriate professional staff are either hired or contracted for the additional services listed below will also be available:

Our Home Care Agency implements programs and services to provide comprehensive, quality home care in the client’s home.

All services are provided under the direction of the Clinical and Executive Administrators and the Director of Nursing (DON). We address the client's unique psychological, safety, self-esteem, and self-actualization needs in a client plan of home care.

Divine Home Health Care, LLC serves as a resource for the client, the client's family, and the community. We provide health care education for the client, the client's family and the community. The agency will directly provide or arrange for skilled nursing services, provided by Registered Nurses and Licensed Practical Nurses (if hired by the agency) and para-professional services, provided by Certified Nursing Assistants. Similarly, services provided by Physical, Occupational and Speech Therapists, Medical Social Workers will be provided to the extent that such professionals are either hired by the agency or contracted for by the agency.

We implement methods of screening applicants and evaluating their progress regularly in order to be able to provide quality personnel.

Divine Home Health Care, LLC provides staff developmental opportunities and programs, including orientation and programs concerning current practices in nursing and allied health care.

Divine Home Health Care, LLC also provides current and accurate personnel records, including verification of current license/certification and health records.

Divine Home Health Care, LLC maintains open communication with clients, employees, and physicians via frequent contact in order to achieve quality and cost-effective care.

The counties that we serve are Cities of: Chesapeake, Virginia Beach, Norfolk, Portsmouth, Hampton, Newport News, Poquoson, Suffolk, Williamsburg, York, James City, Smithfield and Carrollton.

Divine Home Health Care, LLC provides directly or arranges for clinical services from Registered Nurses (“RNs”), Licensed Practical (“LPNs”), and Certified Nursing Assistant (“CNAs”). And either through its own staff or through contract, Physical Therapists (“PTs”), Occupational Therapists (“OTs”), Speech Therapists (“SPs”), and Medical Social Workers (“MSWs”), and Companion care can also be provided.
